Patient records get trapped in digital silos, scattered across different hospitals, specialist offices, and labs. The result? Delayed care, redundant tests, and clinicians forced to make high-stakes decisions with an incomplete picture.
This is the reality of fragmented medical data.
Recognizing this critical gap, Health Link collaborated with Digital Dialogue on the Data Exchange initiative, paving a way towards a future of connected care. Our team had the privilege of collaborating with them to bring this vision to life by building the technological backbone for Health Link, a revolutionary platform that creates a single, unified source of truth for patient records.
The Challenge
Overcoming Data Silos
Patient records, from diagnoses and lab results to allergies and prescriptions, were locked away in the disparate hospitals and systems of various Healthcare Providers. For a consulting doctor or an emergency room physician to get a holistic view of a new patient, they had to embark on a slow, manual process of requesting records from multiple institutions.
This process caused critical delays that could directly impact the quality and speed of care.
How we Innovated and Delivered
Blueprint for Data Integration
Our mission was to engineer a solution that could seamlessly and securely connect these isolated data points. Working hand-in-hand with Health Link team, we developed an integration, streamlined strategy to power the Health Link platform.
- Data Aggregation: Health Link securely pulls patient medical records from a network of participating healthcare providers with DDLG’s secure data integration capability.
- Standardization on FHIR: Next, we tackled the data “language” barrier. Data from different systems is recorded in different formats. We architected the solution to process and store all collected data on a centralized FHIR (Fast Healthcare Interoperability Resources) Server. Using the FHIR standard is crucial, as it ensures that data from different systems is translated into a universal format, allowing for true interoperability.
- Secure & Authorized Access: Through a secure portal, authorized Healthcare Service Providers can now instantly search and retrieve comprehensive patient records. This gives them a complete, holistic view of a patient’s health history at the point of care.
The Impact

The launch of the Health Link platform has been transformative for both clinicians and patients.
- Empowered Doctors: Physicians are no longer working in the dark. With permission-based access to a complete medical history, they can make more informed diagnoses, prevent adverse drug reactions, and avoid ordering redundant tests. This saves valuable time and resources, allowing them to focus on what matters most: the patient.
- Enhanced Patient Safety: Patients now benefit from a more coordinated and safer healthcare experience. In an emergency, first responders and doctors can immediately access life-saving information, such as allergies or pre-existing conditions.
- A More Efficient Ecosystem: By eliminating the administrative friction of chasing down records, Health Link creates a more efficient and collaborative healthcare system for everyone involved.
Our collaboration with Health Link was about more than just a technological success, it was about helping re-architect a system to be truly patient-centric. ความท้าทาย: ข้อมูลที่กระจัดกระจาย (Data Silos)
ปัญหาหลักคือการไม่สามารถเข้าถึงข้อมูลได้ ประวัติของผู้ป่วย ตั้งแต่การวินิจฉัย ผลตรวจทางห้องปฏิบัติการ ไปจนถึงประวัติการแพ้ยาและใบสั่งยา ถูกจัดเก็บแยกกันอยู่ในระบบที่แตกต่างกันของสถานพยาบาลแต่ละแห่ง การที่แพทย์ที่ให้คำปรึกษาหรือแพทย์ในห้องฉุกเฉินจะมองเห็นภาพรวมสุขภาพของผู้ป่วยรายใหม่ได้อย่างครบถ้วนนั้น พวกเขาต้องเริ่มต้นกระบวนการที่ช้าและต้องทำด้วยตนเองในการขอประวัติจากสถาบันหลายแห่ง ซึ่งก่อให้เกิดความล่าช้าที่อาจส่งผลกระทบต่อคุณภาพและความเร็วในการดูแลรักษา
